Fragebogen zur erstmaligen Meldung der Heilberufe – “Initial Registration Questionnaire for Medical Professionals” – registration form for a Jewish female medical professional in the Rohatyn area of the Galicia district under Nazi German rule. The form includes her personal details and passport photograph, identifying her as Lupank, a “Polish Jew”, born in Jarosław (1908), a certified nurse who worked at the hospital in Rohatyn. The form is printed in four languages: German, Polish, Russian, and Ukrainian, and is marked with a red Star of David and a swastika stamp. Dated: July 6, 1942.

This was a mandatory document that the Nazis required Jewish doctors, pharmacists, nurses, midwives, and other medical professionals to submit, with the aim of registering and closely monitoring them through bureaucratic control. The form bears the official stamp: Der Kreishauptmann im Distrikt Galizien – District Commissioner representing the German authorities in the Galicia district. It includes the letter H (Heilberuf) – classifying the registrant as a medical professional. The form also demands detailed genealogical information: the names of the paternal grandfather and grandmother, maternal grandfather and grandmother, maiden name, and poses questions such as: “Did you have a license to practice your medical profession in Soviet territories?” “Did you serve in the Red Army until July 1, 1941?” and others.
